Job Summary
The Transplant Social Worker provides pre-transplant patient education and emotional support, from date of initial referral through transplant hospitalization and for the duration of the post-transplant period.  They are responsible for referrals to community agencies, coordinating patient care with other team members, and assisting with concrete services.  All patient’s seen and evaluated by Transplant Social worker and the team, will be presented at the Multidisciplinary team meeting with all disciplines participation.
 


Qualifications


·       Masters degree in social work.

·       STSW membership required in good standing

·       Certified Clinical Transplant Social Worker (CCTSW) credential is required within 3 years of joining Transplant Team

·       LCSW required

Experience:

·       A minimum of two years of social work experience in a hospital or health care setting

·       Transplant social work experience preferred.

·       Excellent interpersonal and written communication skills

·       Current clinical assessment skills

·       Ability to act in an autonomous, self-directed manner.
