Minimum Requirements
- Bachelor of Science in Physical Therapy from an APTA-approved program, or equivalent entry-level degree required.
- Current, active Oregon Physical Therapist license required prior to the start of the assignment.
- Proof of current CPR certification required.
- Current driver’s license, reliable transportation, and personal auto insurance required.
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
- Previous home health experience preferred.
- Ability to perform clinical assessments, develop and revise Plans of Care, document progress notes, and collaborate with interdisciplinary teams as part of routine practice.
- Ability to lift, bend, push, pull, and perform prolonged standing and walking as required for home health visits.
- Ability to travel locally and work a flexible schedule.
- Fluent English reading, writing, and speaking skills required.
