Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Arizona State Physical Therapy license required to start the assignment.
• BLS (Basic Life Support) certification from American Heart Association or American Red Cross may be required.
• Experience working in a skilled nursing facility or long-term care setting is required.
• Experience providing care to geriatric patient populations, including restorative and maintenance programs.
• Demonstrated ability to document skilled treatments, develop individualized care plans, and work within interdisciplinary teams.
• Client requires availability to work a 4x8 schedule, 5 days per week, with a 32-hour weekly guarantee.
• Preferred: Prior travel therapy experience and familiarity with common SNF equipment (Hoyer lift, gait belts, parallel bars). Preferred items are not required.

• Facility Type: Skilled Nursing Facility.
• Shift Name: Days.
• Schedule: 4x8, Monday through Friday (days), 32 hours per week guaranteed.
• Assignment Length: 13 weeks.
• May be required to float to other nearby facilities as needed.
