Physical Therapist - Travel Contract

Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Arizona State Physical Therapy license required to start the assignment.
- BLS (Basic Life Support) certification from American Heart Association or American Red Cross may be required.
- Experience working in a skilled nursing facility or long-term care setting is required.
- Experience providing care to geriatric patient populations, including restorative and maintenance programs.
- Demonstrated ability to document skilled treatments, develop individualized care plans, and work within interdisciplinary teams.
- Client requires availability to work a 4x8 schedule, 5 days per week, with a 32-hour weekly guarantee.
- Preferred: Prior travel therapy experience and familiarity with common SNF equipment (Hoyer lift, gait belts, parallel bars). Preferred items are not required.
Assignment Details
- Facility Type: Skilled Nursing Facility.
- Shift Name: Days.
- Schedule: 4x8, Monday through Friday (days), 32 hours per week guaranteed.
- Assignment Length: 13 weeks.
- May be required to float to other nearby facilities as needed.
